Experience Base – System Safety and Process Hazard Analysis
Members of Kazarians & Associates, Inc. project team have been conducting system safety and process hazard analyses since 1979. Collectively, we have analyzed several hundred systems and processes from practically all industries. The following is a summary of this experience including some of the noteworthy projects:

• Conducted HAZOP studies for practically every process unit in a large oil refinery for several major refineries.
• Conducted Process Hazard Analysis as an integral part of every PSM, RMP and CalARP project. In these projects, processes used in chemical manufacturing, water treatment, wastewater treatment, ammonia refrigeration, electroplating, tank car maintenance, oil and gas production, and others, have been addressed.
• Conducted a thorough system safety analysis of a very large hydrogen braze furnace using the FMEA methodology.
• Have developed fault trees for systems supporting nuclear power production.
• Assisted a testing facility in identifying potential explosion scenarios involving the operation of a test boiler.
