As an integral part of all safety programs developed for our clients, Kazarians & Associates, Inc. provides an incident investigation procedure that is tailor-made to the specific management and facility conditions of our clients. In addition to developing the procedures, we have assisted in their implementation. The following is a summary of our experience:

• Kazarians & Associates, Inc. has developed incident investigation procedures for a wide range of facilities including water and wastewater treatment, food processing, manufacturing, chemicals handling, etc.
• Kazarians & Associates, Inc. has assisted several clients from the water treatment, wastewater treatment, and parts manufacturing industries, in conducting incident investigations, reaching proper conclusions, and developing a workable plan to minimize the likelihood of recurrence.
• Dr. Kazarians assisted the personnel of a major refinery in investigating the root causes of a valve failure that lead to a hydrocarbon spill and a release of trace amounts of toxic gases.
• Dr. Kazarians provided analysis support in investigating the vulnerability of tank cars to vandalism as part of an incident investigation process involving a similar event.
• Our project team assisted the legal team investigating a fatality event at an explosive materials handling and manufacturing plant in assessing the root programmatic causes of the incident.
