To complement the pro-active safety programs and procedures that anticipate safety situations, the accident and incident investigation process provides the means for learning from actual events. This process allows facility management to identify the underlying causes of incidents and plan for steps to prevent similar events. An important element of this process is the recognition and analysis of “near miss” events. As it is clearly demonstrated by experience, management of near misses is an effective means for reducing unwanted incidents.
